Before booking your VIDEO APPOINTMENT, you must have the following ready:

A Computer
You will require a COMPUTER or SMARTPHONE with a VIDEO CAMERA and a MICROPHONE

OHIP Card
A VALID ONTARIO health card. If you DO NOT have a Valid OHIP card, select a PAID visit.

A Fax Number
Have your pharmacy's FAX NUMBER ready for us to fax any prescriptions.

A Credit Card
A Credit Card (no visa debit or debit cards) for any additional charges (e.g. notes)
Please Note: Virtual Care/Video Appointments are covered by OHIP only if you have a VALID OHIP card AND have been seen by our physicians once in the past 2 years. EXCEPTIONS include MEDICAL ABORTIONS and CONSULTS from another physician (referral letter required). You may be asked to be seen by one of our physicians following your appointment to qualify for this service.

(Any errors will be carried over to forms and test requisitions leading to unnecessary delays)

(If you experience difficulties with a VIDEO call, we may contact you via a 1-800 phone number)

(This is required to complete your VIDEO appointment with the physician)

(No show and missed appointments will be charged a $50 fee)

FAQ
Yes, this platform has been approved by the Ministry of Health and the Ontario Medical Association as a secure and valid form of communication between physicians and patients.
Yes. A $50 hold payment fee applies to expired healthcards (refundable upon presenting an updated/valid healthcard.
Yes, you may still see a physician, but you will be required to book a PAID ($75) visit.
At this time, we do not accept UHIP or any other third party insurance coverage. You can still book a PAID ($75) visit, and then submit the receipts from your visit to your insurance company. However, at this time we cannot guarantee that the cost will be covered by your insurer. We encourage you to check ahead of time.
We can see patients for medical abortions, initial assessments, follow up test results, cough/colds, UTI, prescription refills, referrals, and mental health concerns. Please note that we cannot refill or prescribe restricted medications (narcotics, benzodiazepines, or ADHD medications).
Yes. However, the parent/guardian must be present with any child under the age of 14 during the video visit.
Prescriptions for Narcotics, ADHD meds, benzodiazepines, EMERGENCY CARE (chest pain, stroke like symptoms, sudden shortness of breath), completion of 3rd party forms (WSIB disability forms, attending physician statements).
For patients requiring form completion, we can schedule you an in-person appointment during the daytime OPTIONS Clinic.
Yes. We can send a sick note by email ($25 fee). Specialized notes such as for Laurier University and University of Waterloo can be issued, but not emailed. Students requiring notes will need to pick them up at the clinic, as they cannot be sent electronically at this time.
Yes, however you may be asked to be seen in person by a physician before you can book future appointments. OHIP now requires that patients be seen in person (walk-in clinic or a scheduled daytime appointment) by the same physician every 2 years to qualify for Virtual Care.
